Forms and applications can be submitted below through the provided links to the MEYHA Board of Directors.

Please ensure all forms are completed in full and submitted promptly.

If you believe your player should be placed on a team above the level recommended by their birth year, please complete this form. Your request will be reviewed by the MEYHA Board of Directors.

Players will continue to participate in practices and games at their current registered level until a decision is made.

If the request is approved, the President will contact you, update the registration, and adjust the fee according to the new level of play.

Please complete this form by December 1st.
